Swollen Ankles

Ankle swelling, medically termed peripheral oedema, develops when excess fluid accumulates within the soft tissues. The lower legs are particularly vulnerable due to gravity and the distance blood must travel back to the heart.

A balance between governs fluid movement within the body:

  • Blood pressure within small vessels
  • Oncotic pressure generated by plasma proteins

  • Venous return efficiency

Disruption to any of these systems can lead to swelling.

Common causes include prolonged immobility, pregnancy, obesity, certain medications such as calcium channel blockers, and systemic conditions including heart failure, kidney impairment or liver disease.

However, in vascular practice, one of the most frequent causes is chronic venous insufficiency. In this condition, faulty venous valves allow blood to flow backwards and pool within the lower limbs. The sustained increase in venous pressure alters fluid exchange at a microscopic level, allowing fluid to leak into surrounding tissues.

Importantly, ankle swelling is a symptom. A thorough clinical assessment is required to determine the underlying cause.

Varicose Veins

The venous system in the legs consists of deep veins, superficial veins and perforator veins that connect the two systems. One-way valves within these veins ensure blood flows upward towards the heart.

When valves weaken, blood refluxes. This backward flow raises pressure within the superficial veins, causing them to stretch and become tortuous. The visible, bulging veins beneath the skin are the outward sign of this process.

Valve failure most commonly affects the great saphenous vein or small saphenous vein. Over time, reflux may extend into tributary veins, increasing venous hypertension throughout the lower leg.

This raised pressure has consequences beyond the visible veins. It alters the microcirculation in surrounding tissues, setting the stage for swelling and skin changes.

Venous Hypertension and Tissue Fluid Accumulation

Persistent high pressure in the venous system is called venous hypertension. It is central to the development of ankle swelling in patients with varicose veins.

At the capillary level, elevated venous pressure reduces the reabsorption of interstitial fluid. Instead of returning to circulation, the fluid remains within tissues. In the early stages, the lymphatic system can compensate. Over time, lymphatic drainage becomes overwhelmed.

This process leads to:

It tends to worsen after prolonged standing or sitting and improves with leg elevation. Symptoms frequently include aching, throbbing, heaviness or itching. Visible varicose veins are commonly present, though not always.

As the condition advances, further changes may appear:

  • Hyperpigmentation around the inner ankle
  • Dry, inflamed or itchy skin
  • Thickening and hardening of the lower leg
  • Tenderness over areas of inflammation
  • Development of superficial thrombophlebitis

In severe cases, skin breakdown may occur, resulting in a venous leg ulcer. These ulcers typically develop near the medial malleolus and can be slow to heal without addressing the underlying reflux.

Sudden swelling affecting one leg only, particularly if associated with pain or redness, requires urgent medical review to exclude deep vein thrombosis.

Differential Diagnosis

Not all ankle swelling is venous in origin. Accurate diagnosis requires careful consideration of alternative causes.

  • Cardiac-related swelling is often bilateral and associated with breathlessness.
  • Renal causes may present with swelling elsewhere, including the face.
  • Lymphoedema produces firm, non-pitting swelling that may extend to the toes.
  • Medication-induced oedema typically develops after starting new treatment.

Distinguishing between these causes is important, as management differs significantly.

Vascular Assessment

Clinical examination alone cannot fully map venous dysfunction. Duplex ultrasound scanning is the gold standard investigation for suspected venous insufficiency.

The scan provides:

  • Visualisation of vein anatomy
  • Measurement of blood flow direction
  • Identification of reflux duration
  • Assessment of the deep venous system
  • Detection of previous thrombosis

This detailed imaging allows precise localisation of valve failure. It also ensures that deep veins are functioning properly before superficial treatment is planned.

A structured assessment enables targeted treatment rather than symptomatic management alone.

Conservative Management

In early disease or where intervention is not immediately indicated, supportive measures may reduce symptoms.

Graduated compression stockings apply external pressure, encouraging venous return and reducing fluid accumulation. They are particularly helpful for patients who stand for long periods.

Regular walking activates the calf muscle pump, which helps blood flow back up. Elevating the legs during rest can reduce evening swelling. Weight reduction may lower venous pressure in individuals with obesity.

While these measures improve comfort, they do not correct underlying valve failure.

Minimally Invasive Vein Treatments

Modern varicose vein treatment aims to close the faulty vein responsible for reflux. These procedures are typically performed under local anaesthetic as day-case treatments.

  • Endovenous laser therapy delivers controlled thermal energy within the vein, sealing it closed.
  • Radiofrequency ablation uses heat generated by radiofrequency waves to achieve a similar effect.
  • Cyanoacrylate glue seals the vein without heat and does not require tumescent anaesthetic.
  • Foam sclerotherapy involves injecting a sclerosant that collapses the vein lining.

Disease Progression Without Intervention

Chronic venous insufficiency is progressive. Persistent swelling can lead to tissue fibrosis, making oedema more permanent. Inflammatory changes may worsen, leading to lipodermatosclerosis, in which the lower leg becomes firm and tender.

Venous ulceration represents the most advanced stage. These wounds are associated with prolonged healing and a higher risk of recurrence if reflux remains untreated.

NICE guidelines recommend referral to a vascular service for patients with symptomatic varicose veins, skin changes related to venous disease, superficial vein thrombosis or venous ulceration.

Early treatment is associated with better clinical outcomes and reduced long-term complications.

Pregnancy and Venous Symptoms

Hormonal changes during pregnancy relax vein walls and increase blood volume. Pressure from the enlarging uterus impairs venous return from the legs. As a result, varicose veins and ankle swelling are common, particularly in the third trimester.

Most swelling resolves after delivery. However, persistent varicose veins should be assessed if symptoms continue beyond the postnatal period.
