Prevalence of Varicose Veins

Bonn Vein Study

One of the most extensive studies on venous disease is the Bonn Vein Study, conducted by the German Society of Phlebology between 2000 and 2002. This study analysed over 3,000 participants who completed a questionnaire and underwent an exam, including an ultrasound scan. Participants were categorised into seven classes ranging from C0 (no vein abnormality) to C6 (open leg ulcers).

The study found that less than 10% of participants had normal legs with no varicose veins, spider veins, or abnormalities detected by ultrasound. About 20% of people had varicose eczema, ulceration, or swelling, and approximately one-third had superficial venous insufficiency affecting blood circulation in the legs. Fourteen per cent of people fell into the C2 category, having varicose veins without any skin changes or oedema, including 12% of men and 16% of women.

Edinburgh Vein Study

The Edinburgh Vein Study, another significant study on venous disease, began in 1999 with invitations sent to almost 1,500 men and women. This study involved a 13-year follow-up exam, during which participants completed a questionnaire about their medical and lifestyle history, followed by an exam to classify venous disease.

After 13 years, the follow-up exam and reclassification aimed to determine if participants had new or progressing varicose veins or complications. The study found that nearly 50% of participants with venous disease had deteriorated by the follow-up—roughly one-third of those with varicose veins experienced complications. The incidence of varicose veins was 18%, with about 10% of people aged 18 to 34 and almost 26% of those aged 55 to 64 affected.

 

Factors Influencing the Incidence of Varicose Veins

  • Age

The Edinburgh study shows that the incidence of varicose veins increases with age. While about 10% of younger adults have varicose veins, the risk significantly rises after age 50, with nearly half of all seniors experiencing varicose veins. As you age, veins lose elasticity, causing them to stretch and the valves to weaken. This results in blood pooling in the veins, leading to varicose veins.

  • Obesity

The United Kingdom is one of Western Europe’s most overweight nations, with an obesity rate growing faster than in the United States. About 27% of people in the UK are obese, and another 36% are overweight. Being overweight increases pressure on the leg veins, causing the valves to weaken and fail. Losing weight can reduce the risk of developing new varicose veins but cannot eliminate existing ones.

  • Gender

Studies, including the Edinburgh and Bonn vein studies, have found that women are more likely to develop varicose veins than men. This may be due to higher progesterone levels in women, which relaxes blood vessel walls and can weaken the valves. Pregnancy also increases the risk of varicose veins due to higher progesterone levels, decreased blood flow from the legs to the pelvis, and increased blood volume. Up to half of all pregnant women may develop varicose veins.

  • Heredity

Genetics play a significant role in the development of varicose veins. If you have close family members with varicose veins, your risk of developing them is higher.

Sclerotherapy
A solution is injected into the vein, causing it to scar and close.
